Medicaid Waiver Social Worker Case Manager

General Statement of Duties:
This is a systematic process of service planning, monitoring, and follow-up to properly meet the needs of individuals who meet the requirements for the Medicaid Waiver Program.

Responsibilities:

The case management team, composed of a licensed social worker and registered nurse, will work together as a team and will jointly be responsible for the following activities:
  • Responsible for assessment of the assigned client, developing and initiating an appropriate plan of care, arranging for the provision of services regardless of payor source, and for monitoring each plan of care;
  • Coordinates the efforts of family, friends, or volunteers to provides services to clients;
  • Contacts potential service providers to negotiate delivery of services, prepares written referrals to community service agencies, explores the availability and quality of services and eligibility criteria, and the accessibility of services to the client;
  • Arranges for, and attends case conferences as needed;
  • When appropriate, assists clients and support systems on a short-term basis;
  • Maximizes and coordinates appropriate informal and community resources;
  • Monitors and reviews continued appropriateness of care plan, making revisions where necessary; visits in the home of the client at least monthly by at least one member of the team, and by both members of the team each quarter.
  • Maintains complete documentation of clients’ progress and his/her interaction with service providers, according to the case management documentation standards; completes all applications, forms, and additional documentation as required;
  • Provides follow-along to ensure quality of care with case reviews that will focus on the individuals progress in meeting goals and objectives established through the plan of care.
  • A Case Manager shall maintain an average, active caseload of no more than 50 clients for a total of 100 clients for the Case Management Team.

Qualifications:


Education:

A social worker licensed to practice in the State of Mississippi with a bachelor’s degree in social work and at least two (2) years of full-time experience in direct services to aged and disabled clients; or A licensed social worker with a bachelor’s degree in a related field and two (2) years full-time experience in direct services to aged and disabled clients.

Knowledge:

  • Thorough knowledge of the principles, procedures, and techniques of professional social work;
  • Knowledge and understanding of psychological, social, health, and economic factors influencing the attitudes and behavior of individuals and families;
  • Knowledge of community resources available to individuals and families, and an interest in mobilizing the specialized function into a more coordinated and comprehensive system;
  • Knowledge and skill in interviewing and assessment techniques;
  • Ability to work in a team relationship.

Training:

  • Case management supervisors and case managers are required to complete the following training:
  • All new case management staff must receive agency induction training and in-service education, or staff development as required by the Case Management Provider.
  • All case management staff are mandated to participate in orientation and training activities scheduled by the Division of Medicaid Waiver. Orientations shall include:
    1. Introduction to the agency and the organization of the Medicaid network;
    2. Overview of the program history, intent and target population;
    3. Introduction to community resources;
    4. Review of agencies and services, policies, procedures, and applicable service regulations.
  • New case management staff shall participate in a program orientation by the Case Management Provider and a minimum of two day field training experience under the supervision of a case manager at the designated training site.
  • Agencies providing case management must have a system in place for identifying the training needs of staff. This training is provided to help staff in performing their case management activities.
