Nutritionally balanced meals delivered to the homes of those unable to leave home without assistance. Persons who are homebound and are receiving case management services are given first priority for home delivered meals. In order to qualify for this program individuals must receive a level II score of 22 or above on the Division of Aging & Adult Services screening instrument.
