Outreach is the extending of services or activities beyond the current or usual limits, including a variety of intervention activities designed to reach out, search for, and identify targeted groups of older individuals (60 years and older), making one-to-one contact to assess their service needs, to inform and encourage their use of available resources and make appropriate referrals.
